Chronic Care Management
A Chronic Care Management pilot program that got under way in January 2007 will identify high-risk and expensive chronic conditions in Medicaid as they emerge. The idea is to match the enrollees with medical homes, ensuring that those patients will get faster and more appropriate care -- and save the state money in the process. The pilot will use modern “predictive modeling” software to select candidates for enrollment in the program.
