CLIENT INFORMATION FOR GAU MANAGED CARE CLIENTS


Here is some information and materials used with the GAU managed care program.  Note: you first need to become eligible through the Community Service Office (CSO).  You can obtain an initial application online.

1.         Once you are approved for the GAU program, you get a Services Card.  You can use it to get medical care.  For the first month you receive GAU, you can use your Services Card to go to any medical provider who will accept DSHS Medical payments. 

2.         In your second month of GAU (in most cases) you will be enrolled into Community Health Plan (CHP).  You then just use CHP providers.  If you do not have a Primary Care Provider (PCP), you need to pick a PCP and clinic that will provide all your medical care, or refer you to care.  You can do this by calling CHP at 1-800-440-1561 or visiting CHPs website at http://www.chpw.org/en/index.php  as soon as you get on GAU.  If you have not yet been enrolled, tell CHP that you are on GAU and would like to get a PCP for when you are enrolled.

3.         If you are on GAU and already have a PCP and/or clinic, then let the customer services staff from CHP know when you contact them.  If your PCP and clinic are already contracted with CHP, you should be able to remain in their care. 

  • If you need mental health services, call your PCP.
  • There are care coordinators at most PCP's clinics who will determine the best services available to you.

  • Call CHP at 1-800-440-1561 or visit their website at http://www.chpw.org/en/index.php for a list of providers, clinics and assistance in choosing a PCP and clinic where you will receive your healthcare. 
  • You can also request the CHP GA-U Member Handbook and list of pharmacies when you contact CHP for your PCP and clinic.
  • The CSO will send you a Welcome to Washington State GAU letter.  It is also important information on managed care.
  • CHP will also send you a Welcome to Community Health Plan letter with important information on your benefits.
