Instructions for Requesting Paper Copies of Provider Documents

Effective November 10, 2008, to request a paper copy, you no longer contact the Department of Printing Fulfillment Center.  Instead, contact DSHS using one of the following methods:

• Internet:  http://hrsa.dshs.wa.gov/contact/MailForm.aspx?type=Provider.  When completing the form, follow these instructions:

• Type in your name.

• Select either "email" or "phone."

• Type in your email address or telephone number.

• Make sure "General information about Health and Recovery Services" is selected.

• In "Subject" type the name of the document you want.

• In "Message" type in the address you want DSHS to send the document to.

• Click on "Send Message."

• Facsimile:  1-360-725-2144.  Please include the following in your fax:

• Your name and provider number.

• The name of the document you would like mailed to you.

• The address you want DSHS to send the document to.

• Telephone:  1-866-562-3022, Option 2.

(Orders take up to one week to fill.)
