Q:
Application Agent: Do I need to use barcodes to send in a potentially eligible
customer?
A: Yes, to receive credit for the Application Agent children’s medical
outreach program the contractor’s unique vendor code barcode label must be
affixed in the designated location found only on application form types: 14-380
– Apple Health for Kids or 14-001 – DSHS Application for Benefits. Without
proper placement of the vendor code barcode label, DSHS will be unable to trace
approved children’s medical applications submitted for this contractor for
compensation purposes.
